This policy explains what NutriGuide Indonesia records when you use this website and the consultation intake form, why we keep it, and how you can ask us to correct or remove it.

NutriGuide Indonesia runs this website and the online nutrition consultation service offered through it. The operator of the site can be reached at Jl. Data we collect

Two kinds of information arrive at this site: what you choose to type, and what your browser sends automatically. Both are described below, and neither is used for advertising.

Information you enter

Your name, contact details, and the goal you describe for your consultation.

Eating and activity notes

Your described eating pattern and your usual activity level, in the words you use yourself.

Health and supplement details

Anything you volunteer about your health, supplements, or medication. This part is optional.

Basic technical data

Browser type, the pages you open, and roughly when you visited — collected so the site keeps working.

If a field makes you uncomfortable, leave it empty. A dietitian can work from a shorter intake, though fewer details can mean a longer first conversation before any guidance is useful.

Why we collect it

Your form data is used to match you with a dietitian, to prepare before your consultation, and to keep a record of the advice given so a later session can start where the last one ended instead of beginning again.

Technical data tells us whether the site is loading properly and which pages people actually read. We look at it in aggregate — how many visits a page received this month, not who visited it. It is never joined to your intake form to build a profile of you.

We do not use your details to sell you anything, and we do not pass them to advertising networks. If a future feature needed different data, we would update this policy first and describe the change here.

Legal basis and consent

Consent when you submit a form

When you send the intake form, you consent to us using those details to arrange and deliver your consultation. Consent covers the consultation itself and the record-keeping that follows it.

Withdrawing that consent

You can withdraw consent at any time by contacting us using the details further up this page, by phone or by email. We will stop using your data, though withdrawing may end a consultation that is still running.

Shared with people, never with advertisers

The list below is short on purpose. Only the people who need your details to do their job ever see them.

Your assigned dietitian

The practitioner preparing your consultation reads your intake before you speak.

Booking coordination

Administrative staff who arrange appointments can see your name and contact details.

Where the law requires it

We may disclose data if Indonesian law obliges us to, and we will tell you when that happens unless we are barred from doing so.

We do not sell your data and we do not share it with advertisers. Nothing you type into the intake form is used to build audiences, retarget you, or feed a marketing list.

How long we keep things

Retention follows a simple idea: keep what a future session needs, remove what it does not.

Active consultations

Your consultation record is kept while your case is active.

After your case closes

We keep the record for a period afterward so a return visit can continue rather than restart.

Forms that lead nowhere

Submissions that do not turn into a booking are deleted within a reasonable period.

Asking us to delete sooner

You can request deletion before that period ends; we will confirm when it is done.

Security

We use reasonable technical and organisational measures to protect the data you provide, and access inside our team is limited to the people who need it for your consultation. We would rather be honest than reassuring: no transmission over the internet can be treated as entirely secure, and we cannot promise that unauthorised access will never occur. If a breach ever affected your details, we would tell you what we know.

Practical protection on your side

Use a device you control when you fill in the intake form, and avoid sending the same health details through more than one channel. If you are unsure whether a message came from us, call the office number above before replying.

Your rights

You may ask what data we hold about you, ask us to correct anything that is wrong, and request that we delete it. Send the request using the contact details at the top of this page and we will respond within a reasonable period. We may ask one or two questions to confirm the request is really yours before we act on it.

  • Correction of any detail that has changed since you submitted it
  • Deletion of your record, subject to any legal duty to keep it

Cookies and analytics

This site uses cookies for the purposes set out in the cookie policy, and you can control them through your browser settings. Blocking every cookie may stop parts of the site from working as intended, including form handling.

Children's data

This site is intended for adults. Where a consultation concerns a child, the parent or guardian provides the information and consents on the child's behalf, and the same rights above belong to that guardian.

Changes to this policy

We may update this policy as the service changes. The current version is always the one published on this page, and the wording you read here is the one that applies when you use the site. If a change affects how your consultation data is used, we will make that clear rather than leave it buried in a reworded paragraph.

Questions about a specific point in this policy are welcome. The same address, phone number, and email at the top of the page reach the people who actually handle intake records — there is no separate department to track down.
